Public Health

Our public health program consists of providing accessible and free of cost linkage to care for individuals living with HIV and individuals uninsured.

 

Survivors Pathway is committed to interrogating the social determinants of health that impact the day to day living of marginalized communities. We build with violence prevention organizations to combat abuse plaguing our Latino, LGBTQIA+, and Black communities. In that framework, Survivors Pathway centers transformative justice in our violence prevention work, empowering and working with survivors.  

 

Case Management for people living with HIV and uninsured

Our case management support is offered to individuals living with HIV and those who are uninsured. We work with community partners to provide access to medical care to all regardless of socio-economic status. For instance our mammography referral service for women uninsured and other basic physical check-ups.

Linkage to care for people newly diagonsed

As a federal HIV testing state, Survivors Pathway provides testing to individuals at risk of contracting HIV and provides linkage to care within 72 hours. Our HIV program is based around prevention and education. Educating our communities on the importance of safe sex practices is an important part of our public health advocacy.

Referral for Hormone Replacement Therapy

Survivors Pathway works in collaboration with community agencies to provide HRT services to our trans and gender non conforming members.
